SCOPE: Comprehensive management of newly diagnosed and recurrent glioblastoma (IDH-wildtype, WHO grade 4) in adults. Covers initial presentation and stabilization, neuroimaging evaluation (MRI with gadolinium, perfusion MRI, MR spectroscopy), histopathologic and molecular profiling (MGMT promoter methylation, TERT promoter mutation, EGFR amplification, +7/-10 chromosome changes), surgical planning (maximal safe resection, stereotactic biopsy), standard Stupp protocol (concurrent temozolomide + radiation followed by adjuvant temozolomide), tumor treating fields (TTFields/Optune), recurrence management (bevacizumab, lomustine, re-resection), supportive care (dexamethasone, seizure management), and palliative care integration. Excludes pediatric gliomas, IDH-mutant astrocytomas, oligodendrogliomas, and brain metastases (separate templates).

Special Handling: Cytology requires rapid transport (<1 hour). Flow cytometry requires fresh specimen at room temperature.

Contraindications: Significant mass effect, midline shift >5 mm, obstructive hydrocephalus, posterior fossa mass effect (herniation risk); coagulopathy (INR >1.5, platelets <50K); skin infection at LP site. ALWAYS obtain CT/MRI before LP.


3. TREATMENT

CRITICAL: Glioblastoma management requires multidisciplinary coordination (neurosurgery, neuro-oncology, radiation oncology, neuropathology). Treatment decisions depend on molecular profiling (especially MGMT methylation), extent of resection, age, and functional status.

A diffuse astrocytic glioma is classified as glioblastoma, IDH-wildtype IF:

  1. Histologically shows features of a diffuse astrocytic glioma, AND
  2. IDH1 and IDH2 are wildtype (no mutation), AND
  3. At least ONE of the following molecular features:
